Home Study is issued by the competent authority of the state confirming the possibility for the applicants to be adoptive parents. In the Home Study must be indicated addresses, living and housing conditions (the number of bedrooms, living conditions for the child), biography, family members (the number of persons living together with applicants, relation degree, biological children), attitude to the adoption. Home Study must include recommendations regarding the number, age and health condition of children family desire to adopt. If the Home Study is issued by non-governmental entity (a private agency or social worker), a copy of the license authorizing this entity to conduct pre-adoption reviews must be attached. Note: Attach Social Worker License and/or Agency License, also write age of kids, SDA consider three types of children’s age – kids under 6 y.o., then 6 -9 y.o. and 9-17 y.o. Pls, indicate only that age of the child you desire to adopt. For example, you want to adopt 12 year old child, so in the Home Study age must be indicated between 10-17 y.o. A Ukraine homestudy may only be 4 pages long. Of course, the social worker will have to write an 8 page report for INS and include all information your state requires. Then they should prepare a separate summary homestudy for your dossier. Some agencies charge for this and some don't. The following is a description of a Ukrainian homestudy which, Focus on Children, recommends: PAGES ONE AND TWO Information on family: Name, address, telephone number. Father's name, birth date, place of birth, health, extended family members, educational history, employment, etc. Mother's name, birth date, place of birth, health, extended family members, educational history, employment, etc. Children's names, birth dates, places of birth, health, education, and note whether biological or adopted. Information on any other people who live in the home. Give information on the following: Marriage: date, place, and marriage certificate number. Discuss marriage relationship. References: Number of references and include comments from some of the references. Resources in community: Types of resources of education, recreational, social, medical, etc. Type of child/ren desired to adopt: Age, sex, race, etc. PAGE THREE: Biography of parents : one-half page for each parent. PAGE FOUR: Title: " Report of Living Conditions" This should include a description of the home, whether it is owned or rented, the number of rooms - bedrooms, bathrooms, living areas, garages, and a description of surroundings and town. Discuss the child/ren's sleeping arrangements and how the child will go to school ( walk, or by car or bus). It should include whether the family has a vacation home and its value, etc. It should also include the monthly income and budget, with a statement that this family can comfortably afford to raise a/another child. It should state that health insurance is available for the child. The home study should end with the agency recommendation and approval of the family and the reasons for this approval. This approval should include the number, age and gender of children the family is qualified to adopt. A copy of the license from the agency or the social worker should be attached. Also the following information MUST be indicated in the Home Study: 1. The adoptive parents have never had their parental rights terminated in a court of law. 2. Adopted children have the same rights as biological children.
