The documents will remain valid only
for 1 (one) year from the issue date. But their term must be valid no less than 6 months for the moment of submission to SDA.
NOTE: Date of the
notary power expiration is
not so important.

Prospective adoptive parents submit to the SDA a dossier which consists of such documents:
1. PETITION TO
ADOPT
Original, notarized and apostilled accompanied by Ukrainian translation.
Must be prepared in 2 originals.
Note: Pls write the real age of children. Do not indicate how you met the child you desire to adopt. Just name, DOB and location (orphanage name) if you want to adopt specific kids.

2. HOME
STUDY (advice
only)
Home Study is issued by the competent authority of the state confirming the possibility for the applicants to be adoptive parents. In the Home Study must be indicated addresses, living and household conditions (the number of bedrooms, living conditions for the child), biographic information of adoptive parents, family members (the number of persons living together with applicants, relation degree, biological children, if any), attitude towards adoption. Home Study must include recommendations regarding the number, age and health condition of children family desire to adopt. If the Home Study is issued by non-governmental entity (a private agency or social worker), a copy of the license authorizing this entity to conduct pre-adoption reviews must be attached. Notarized and apostilled accompanied by Ukrainian translation.
Note: Attach Social Worker License and/or Agency License, also write age of kids. SDA considers three types of children’s age – kids under 6 y.o., then 6 -9 y.o. and 9-17 y.o. Pls, indicate only that age of the child you desire to adopt. For example, you want to adopt 12 year old child, so in the Home Study age must be indicated between 10-17 y.o.
3. AGENCY
LICENSE (COPY)
Confirms the right to perform home study and approve prospective adoptive parents. Must be valid at the moment Home Study is issued. Notarized and apostilled accompanied by Ukrainian translation.
4. SOCIAL WORKER'S
EMPLOYMENT LETTER
Confirms the right to perform home study and approve prospective adoptive parents. Must be valid at the moment Home Study is issuedNotarized and apostilled accompanied by Ukrainian translation.
5. I-171H (COPY)
Entrance and permanent residence permit for the adopted child, issued by the competent authority in the adopting parents' country. For American citizens, the Form I-171H, Notice of Approval of Advance Processing issued by the U.S. Citizenship and Immigration Services (CIS) serves as this document.
Note: Sometimes SDA demands to submit confirmation letter from US Embassy if a family wants to adopt child(ren) older than 15 y.o. It means when you want to adopt children of older age, you fill the I-600A (Application for Advance Processing of Orphan Petition) and I-600 (Petition to Classify Orphan as an Immediate Relative) form in the U.S. Citizenship and Immigration Services (CIS) before you apply for an immigrant visa on behalf of an orphan. After that we may receive confirmation from the US Embassy and submit it to SDA with your dossier.

6. EMPLOYMENT VERIFICATION
for adoptive parents
Proof of income for the last 6 months or notarized copy of the income declaration for the previous calendar year
Note: Must be issued for the last 6 months or notarized copy of the income declaration for the previous calendar year.
If someone of the spouses does not work then it must be submitted notarized and Apostilled petition where information is indicated that he/she is homemaker or does not work.

Comment: For American
Embassy in Kiev you may bring either a copy of your most
recent tax return, or your last year's W2 statements, or 2-3
recent pay stubs. Any of these will suffice.While you don't need your tax
forms for your Ukrainian dossier, BUT do bring the last two
years tax returns with W-2 forms as you may need to show
them at the embassy in Warsaw to prove your income. W-2 form
or tax returns and a statement from your employer indicating
your salary. To avoid confusion, please do NOT copy blank
pages in your tax returns.
7. MEDICAL STATEMENTS for adoptive parents
Original, notarized and apostilled accompanied by Ukrainian translation
Medical certificates for prospective adopting parents should be issued on official letterhead of the medical office where the examination took place. Alternatively, the document can bear a stamp/seal of approving clinic/doctor, if available. The doctor's signature must be notarized and be in blue ink.
If prospective parent has any diseases, the doctor must indicate the code of this disease under the code of ICD-10 (International Classification of Diseases)
If letterheads/seals are not available, the SDAPRC requests that a copy of the doctor's medical license be attached to the form. There must be no corrections.
In case if the child is adopted by one of the spouses Medical certificates must be submitted from both spouses.
Comment: This should
not be just a general statement that you are healthy,
but a declaration that you are specifically not
suffering from any kind of psychiatric, communicable,
internal, skin or venereal diseases as well as a
statement that you are not a drug addict, and you are free from AIDS and syphilis.
NOTE: Include the copy of doctor license or make forms on
doctor letterhead.
8. MARRIAGE
LICENSE (COPY)
Notarized and apostilled accompanied by Ukrainian translation. Must be prepared in 2 copies.
9. PASSPORT COPIES
Must be prepared in 2 copies.
10. POLICE CLEARANCES
Original, notarized and apostilled accompanied by a Ukrainian translation.
Police Clearances for each applicant issued by the competent authority (not from local sheriff and from State police office)
SDA made changes into Family Code Law and there is a phrase that it's forbidden to adopt for those persons: "who were convicted of crimes against life and health, freedom, honor and dignity, sexual freedom and personal immunity, civil security, civil order and morality, in the sphere of drug traffic, psychotropic substances, their analogues or precursors, or have outstanding or not cleared in law established order criminal record for crime execution."
Comment:"No criminal
record" statement supplied by a competent authority for each
adoptive parent, attesting to his/her having no criminal
record. Statement obtained from STATE police office.
11. LETTER OF
OBLIGATION
Original, notarized and apostilled accompanied by Ukrainian translation and submitted in 2 originals. There must be 5 points:
- to register their adopted child with the Ukrainian Embassy or Consulate (with indication of the name and address of the diplomatic mission) within one month after the child entered the country.
- to keep Ukrainian citizenship of the adopted child until age 18
- inform the Ukrainian Embassy or Ukrainian Consulate no less than once a year during first three years after adoption and then once in three years till the children turn 18 about the physical and psychological development, as well as health and educational status of any children adopted from Ukraine
- to allow Ukrainian officials or representatives of the Ukrainian Embassy or Consulate to contact and visit adopted children
- to inform the Ukrainian Embassy or Ukrainian Consulate about the address change of adopted child

12. POWER OF

Notarized copy of the Proof of Home Ownership or Rental Document confirming property ownership or renting the house with indication of total space and living space and the number of bedrooms . Notarized and apostilled accompanied by Ukrainian translation.

15. Appointment Letter
Only original, notarized and apostilled, accompanied by Ukrainian translation
COMMENTS:
Here is list of documents for Ukrainian dossier
which can be fastened together and have one apostile.
1) Medical forms (husband and wife) can be bundled together + license of doctor (if applicable).
2) Employment verifications (husband and wife) can be fastened together
3) Home study and license of agency and social worker can be bundle together
4) Police clearance (husband and wife) can be bundle together.
5) Copy of Passports
You don’t need a separate notarization
and apostille for these documents. They can be attached
together, notarized by the same notary and one apostille for
both documents would be enough. If the documents are signed by
different notaries, please, list them both on apostille and
attach one apostille.
NOTE: Agency license and
social worker’s license can be attached to the home study. All
other letters can also be attached in one document. This way
you will only need one apostille for. See explanation below.
